Consent for Veterinary Treatment & Integrated or Alternative Therapy

Vet Connection employs traditional, alternative and integrated medicine practices. This means that traditional standard of care may be blended with integrated/alternative treatments. Integrated and/or alternative treatments may include, but are not limited to: homeopathic remedies, aromatherapy, Chinese herbs, intravenous therapies, nutraceuticals and/or dietary supplements, and food therapy. The decision to use these various modalities may be based on traditional Chinese medicine theories and/or other non-traditional methodologies.

I am the owner or the agent for the owner of the animal described below, and I have the authority to execute this consent. I hereby consent and authorize Vet Connection to perform examinations, diagnostic tests and medical treatment, including the administration of anesthetic drugs, as determined to be necessary in the judgement of the license veterinarian supervising the care and treatment of my pet. I understand that I can request an estimate of anticipated expenses at any time, and I can decline any treatment at any time.

I realize that my pet may not respond nor benefit from integrated veterinary treatment. I also understand that it is important for me to follow the veterinarian’s instructions on monitoring my pet over the course of the traditional western or integrated veterinary treatment and to promptly and fully report to the veterinarian any adverse effects or unusual behavior by my pet.

I further understand that if my pet is seen by another veterinarian while undergoing or having undergone integrated veterinary treatment, that I should fully inform the other veterinarian that my pet is undergoing or has undergone a complementary veterinary treatment and the nature of the integrated veterinary treatment.
